Documentation of Marriage Preparation Hours for the State of Georgia

This form that may be helpful to you in documenting your work with engaged couples for the State of Georgia. As you know, Georgia law now provides for a discount on the marriage licenses of those couples who can provide proof that they have received a minimum of six (6) hours of marriage preparation. In response to your requests, here is a possible format. Some counties will continue to require that their own form be used, but many are accepting variations. Please feel free to copy this one or to design your own. We have been told that the State of Georgia requires that the form be on parish letterhead/ or some other recognized professional letterhead.
